Below you will find various forms and documents you need to establish and administer your Donor Advised Fund account with SDG Impact Fund.
To submit a form, please download a copy to your computer, complete and save it, and then email, mail, or fax it to SDGIF. If sent electronically, your typewritten name is accepted as your signature. The completed, saved form can be emailed directly to a team member at the SDGIF. Alternatively, you may print the form, complete it by hand, and mail the form to SDGIF.
This is the first step towards regenerative stewardship and for purpose impact. With the SDG Impact Fund's Impact Account, the choice and the purpose led stewardship are yours.
When making a gift of any kind (liquid or non-liquid), we recommend that donors complete this form for your and our records. If you prefer please print, complete, and mail or fax your contribution form. We always recommend that you contact a SDGIF team member to assure safe receipt of any gift.
You can always recommend a grant with this form. If you prefer to print, complete, and mail or fax a grant recommendation, you can use this form.
You can always recommend a MRI with this form. If you prefer to print, complete, and mail or fax a grant recommendation, you can use this form.
The SDG Impact Fund sponsors university research and other activities, per our Operating Policy and DAF Program Guidelines, by awarding grants and contracts to charitable organizations and/or auxiliary entities. Grant and contract awards typically provide support for both the direct and indirect costs of conducting these activities.
