Below you will find various forms and documents you need to establish and administer your Donor Advised Fund account with SDG Impact Fund.
To submit a form, please download a copy to your computer, complete and save it, and then email, mail, or fax it to SDGIF. If sent electronically, your type written name is accepted as your signature. The completed, saved form can be emailed directly to a team member at the SDGIF. Alternatively, you may print the form, complete it by hand, and mail the form to SDGIF.

This is Step #2 for opening a donor-advised fund account with SDGIF. With this form we collect pertinent donor/advisor information to help manage your account.
You can allocate (or reallocate, once a quarter) your investment strategy for your account with this form. If you prefer to print, complete, and mail or fax your allocation, or make other changes to your investment options, you can use this form.
When making a gift of any kind (liquid or non-liquid), we recommend that donors complete this form for your and our records. If you prefer please print, complete, and mail or fax your contribution form. We always recommend that you contact a SDGIF team member to assure safe receipt of any gift.
You can always recommend a grant with this form. If you prefer to print, complete, and mail or fax a grant recommendation, you can use this form.
